The Washington Association for Community Health is convening a virtual learning collaborative to support health centers in improving clinical quality measures related to control of hypertension and diabetes.
This training opportunity will provide attendees with the information needed to:
Review trauma-informed care principles.
Plan a trauma-informed outreach effort by interdisciplinary teams.
Implement a registry of patients with uncontrolled hypertension or diabetes.
Use a defined process for trauma-informed outreach (e.g., via phone, mail, email, text message, etc.) to patients with uncontrolled hypertension or diabetes.
